Our Client’s Intermediate Care Service is seeking a dedicated and motivated Social Worker to join their fantastic team in Southwark London Borough.
This 6‑month fixed‑term opportunity has been created to support the service in addressing a waiting list backlog, ensuring residents receive timely, high‑quality support.
Working as part of an integrated Health and Social Care team, you will play a key role in supporting residents through their recovery and reablement journey. You will work closely with multi‑disciplinary colleagues to deliver coordinated, person‑centred care that promotes independence, wellbeing, and positive outcomes.
