Social Worker - In-Patient - Behavioral Health - Second Chance Unit / 3 N

The Second Change Program is a partnership between the NYS Office of Mental Health and NYP.  Its purpose is to improve the care of NY’s long-stay psychiatric population.  This program is focused on those who have had long, continuous hospitalizations because of persistent psychotic symptoms and behaviors that interfere with community living.  Although their illnesses have prevented them from living in the community in the past, these patients have the potential for improvement and eventual discharge from the hospital. Treatment consists of medication, intensive education about mental illnesses and its treatment, social skills training, skills for community living and discharge planning.  The program also addresses drug abuse and other problems that may cause relapse.
