As a Social Worker, you’ll serve as a trusted counselor, crisis responder, advocate, and systems‑thinker—conducting psychosocial assessments, coordinating care, removing barriers, and ensuring every patient receives timely support and a safe, effective plan for life beyond the hospital. You will collaborate across disciplines to improve quality, optimize resources, and keep care moving—so patients feel supported, informed, and never alone in their journey.

Essential Functions

  • Complete comprehensive psychosocial assessments focused on post‑hospital needs for designated patients and their support systems; document findings and recommended interventions.

  • Provide counseling and crisis intervention to help patients and families cope with stressors related to hospitalization, disability, chronic, or terminal illness.

  • Partner with Clinical Care Management to evaluate and place patients in the most appropriate level of care or setting across the continuum.

  • Serve as a clinical resource for clinicians and physicians on the emotional, social, and psychosocial dimensions of illness and their impact on family and community supports.

  • Educate patients and families on community resources and post‑hospital options, tailoring information to age‑specific needs; initiate and track referrals to community agencies.

  • Report suspected child or adult abuse, neglect, or exploitation per policy and law; act as liaison with the Department of Social Services during evaluation and investigation.

  • Respond to consult requests after normal business hours as part of scheduled on‑call coverage to support safe, timely patient throughput.

  • Advance patient throughput and discharge planning by proactively addressing barriers, promoting appropriate length of stay, and supporting efficient utilization of resources.

  • Promote organization‑wide quality improvement, analyzing processes and variances to identify opportunities for enhanced care coordination.

  • Serve as a key information and education resource for the interdisciplinary team, supporting communication, problem solving, and alignment of care plans with patient goals and safety.

  • Contribute to optimal clinical and resource outcomes, ensuring reimbursement alignment and facilitating efficient transitions across the continuum.

Educational Requirements

  • Master’s Degree in Social Work (MSW) — required.

  • Licensure: LCSW preferred; applicable state certification preferred.

  • Professional Standards: Adherence to the National Association of Social Workers (NASW) Code of Ethics.

  • Experience: Minimum of 1 year of professional experience in a hospital or health‑related setting preferred.

  • Technical Skills: Experience with data management tools; proficiency in electronic medical documentation preferred.
