About the Opportunity:
St Vincent's Private Hospital Brisbane is seeking an experienced social worker or discharge planner to join our allied health team. Our spectacular Kangaroo Point location caters for patients with varied and complex needs, including navigating the aged care and NDIS pathways to discharge from hospital.
A snapshot of your duties would include:
Comprehensive psychosocial assessments.
Assist patients and their families in accessing relevant services and funding.
Liaise with external providers to facilitate timely hospital discharges.
Participate in peer learning and support within the department.
Participate in multidisciplinary meetings.

Current registration with relevant body (AHPRA, AASW)
Relevant qualification (Social work, RN, OT)
Minimum of 2 years’ experience working within a hospital facility
Strong professionalism and commitment to person centred care
Demonstrated effective interpersonal and communication skills
Experience making QCAT, NDIS and aged care referrals and applications - strongly desired
