Medical Social Worker
Job Description:
Case management services are designed to assist patient and family members with continuing care arrangements that may be necessary following discharge from the hospital. Case managers work with physicians, nurses, physical therapists and other members of the health care team to identify and coordinate alternative care or services that will assist in recovery.

Job Description:
- The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting.
- This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization.
- The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge.
Job Requirements:
- Master’s degree in Social Work
- Two years job related experience
- Ky CSW license
- Case Management (CCM) certification preferred
