Summary of Job Duties:

The STARLITE Social Work Case Manager is responsible for assisting and providing one-on-one case management support and facilitating small groups for at-risk 7th-12th grade students and their families in the Pine Bluff School District. The STARLITE Social Work Case Manager will coordinate individual academic & behavioral supports for students, including focusing on promoting social-emotional learning, mental health services, and crisis management training for staff to create a safe learning environment within Pine Bluff schools. The STARLITE Social Work Case Manager will collaborate with the Program Manager and School Faculty, and other community-based organizations, to ensure student success and decrease youth acts of violence at Pine Bluff Schools. Responsible for attending weekly meetings to consult on cases and review case plans and direction. Develop and maintain positive networking relationships with community partners and resources for students and families of the Pine Bluff School District. The role reflects appropriate knowledge of case management scope of practice, current state requirements, and other Federal or State regulatory agency requirements specific to the case management role. The STARLITE Social Work Case Manager will partner with the multi-disciplinary team, utilizing best practices, relevant data, and compliance with the mission/philosophy, standards, goals, and core values of UAMS: Comfort, Hope, and Healing. Primary elements of the case management role include: assessing the needs of the student and families, providing information, support, counseling, and referrals to other levels of care and appropriate resources.


Qualifications:

  • Bachelor’s degree in Psychology, Sociology, Education, Social Work, Rehabilitation, or related field, plus two (2) years of social work experience, including one (1) year of experience working with kids or

  • Master's degree in Psychology, Sociology, Education, Social Work, Rehabilitation, or related field, including one (1) year of experience working with kids required.

Responsibilities:

  • Assesses the eligible individual to determine service needs.

  • Makes contact with the student's family, school faculty, and professionals on their behalf. Assessment includes collecting information about the student’s situation and identifying the student’s problems and needs. The assessment is comprehensive and includes medical, social, educational, and other services.

  • Promotes student and family-centered engagement.

  • Recommends care and services that are appropriate to the specified chronological age, developmental age, social maturity, and cultural background.

  • Documents and communicates with all team members of student and family concerns pertaining to the coordination of safety and preventing school violence.

  • Developing or assisting in the development of an individualized safety plan, specific to the student's needs.

  • Provide referrals and coordinate linkages to help the student obtain needed services. This includes contacting service providers selected by the family of the student and assistance in the delivery of services identified in the service plan.  

  • Scheduling appointments related to gaining access to medical, social, educational, and other services appropriate to the student’s needs.

  • Assists in planning and participation in education events to prevent youth violence and enhance school safety, including support groups, workshops, education modules, and communications with persons and families living within the Pine Bluff School District.

  • Performs other related responsibilities as assigned.
