About Us – Social Services

• The Social Services team provides discharge planning to every patient who needs assistance with placements, equipment, services, social issues, etc
• Each inpatient department is assigned a Social Worker. This includes Freeman West, East and Neosho
• Our team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possible

Schedule: Full Time

What You'll Do
Functions as a member of the multi-disciplinary health care team to assess and plan for patients to leave the acute psychiatric care setting. Responsible for completing psychosocial assessments, coordinating discharge plans, needs, and arrangements for patients, and for providing information, education, and linkage with appropriate community resources and services.

Requirements

• Bachelor's degree Social Work (BSW) or Bachelor's degree in a related field.
• Minimum 2 years experience in the Social Work field or in a related area.
• Licensed Social Worker, Therapist or Social Services Provider.

Preferred Requirements

• MSW and/or LCSW.
