In this Home Health LPN role, you will:
Provide skilled nursing care in the home under the direction of an RN and in accordance with the physician’s Plan of Care
Review and follow the Plan of Care prior to each visit to ensure safe, effective treatment
Document patient assessments, care provided, and progress accurately and timely
Communicate changes in patient condition, safety concerns, or environmental risks to the RN Case Manager and care team
Assist patients and caregivers with education on medications, treatments, and self-care techniques
Support patient safety using family and community resources
Collaborate closely with RN Case Managers during normal business hours for guidance and clinical support
Maintain accurate medication documentation and update medication profiles per agency guidelines
Deliver compassionate, respectful care to patients, families, and caregivers
Take proactive steps to help prevent avoidable hospitalizations and emergency room visits
Graduate of an accredited Practical Nursing program (Certificate, Diploma, or Associate Degree)
Active state license as a Licensed Practical Nurse (LPN)
Minimum of 1 year of clinical nursing experience
Home health experience preferred
Ability to travel up to 50% within the assigned territory
Ability to sit, stand, bend, lift, and move intermittently; must be able to lift up to 50–100 lbs
Valid driver’s license, current auto insurance, and reliable transportation
