About the role
Provide physiotherapy assessment and treatment for clients in the community under the Transitional Aged Care Program, working within a friendly and supportive team environment to deliver slow-stream, home-based therapeutic interventions that maximise functional improvement
Contribute to client outcomes through case management, care planning and ongoing evaluation of service needs within a community-based model of care
Deliver clinical supervision and support to junior physiotherapists and allied health assistants to ensure safe, effective practice and professional development
Lead and initiate quality improvement projects within the Transitional Aged Care team, supporting innovation, service development and enhanced client experience
Build strong relationships with community services, health professionals and groups, providing education and contributing to collaboration across care pathways
About you
Current registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a physiotherapist and a Current NSW Class C or above driver’s licence
Experience in advanced assessment, clinical reasoning and evidence-based intervention in the aged care or community rehabilitation setting
Demonstrated ability to provide clinical supervision, support and governance to Level 1/2 physiotherapists, assistants and the wider multidisciplinary team
Demonstrated commitment to improving clinical practice and service delivery through leadership of quality improvement activities and ongoing professional development
Demonstrated effective communication, consultation, negotiation and interpersonal skills
