As Senior Manager Transitional Care, you’ll contribute to our success in the following ways:

  • Oversees ElaraConnect Transition program related to identification of potential hospice care needs across service lines. Identifies best practices to identify and transition patients across care continuum.
  • Develops processes among all departments and disciplines to ensure coordinated, comprehensive quality services to the patients.
  • Oversees the ElaraConnect operations for HOS lead generation, referral qualification, admission conversion and eligibility and admission requirements.
  • Monitors and improves conversion rates for entire process from lead generation through admission.
  • Identifies growth opportunities based on staffing efficiencies or market penetration.
  • Adheres to staffing models and maintains appropriate staff productivity levels.
  • Establishes and implements key quality metrics to evaluate the quality of patient care. Participates in ongoing continuous quality improvement education and training.
  • Ensures appropriate outcomes related data is appropriately collected and available for internal and external use.
  • Reviews financial indicators and implements action and modifications when warranted.
  • Supervises the direction and management of operational staff, including but not limited to training, mentoring, and counseling, as appropriate. Works with and advises managers to improve and correct performance.
  • Performs other duties/projects as assigned.

What is Required?

  • Bachelor’s Degree in Business Management or related field is required.
  • 5+ years’ experience in home health or hospice with experience in Medicare/Medicaid, Home Care Benefits, Policies and procedures, and compliance requirements.
  • 5+ years’ experience in a successful leadership role.
  • Current and unrestricted RN license
  • Ability and willingness to travel approximately 25% for meetings, education, and business needs.

You will report to the Vice President ElaraConnect or Director of Care Transitions.
