POSITION SUMMARY:

The RN Transitional Care Manager will care manage and support the transition of complex, chronically ill clients in alternative care settings to home while educating referral sources about BAYADA Home Health Care; building referrals and long-term relationships.

This is a hybrid role, with regular travel required through Southern NJ, DE, and Southeast PA.

MINIMUM QUALIFICATIONS:

  1. Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability.
  2. Four (4) year college degree
  3. Be a licensed Registered Nurse.
  4. Five (5) years experience in nursing, discharge planning or home care in a respective specialty (pediatrics, rehabilitation, etc.).
  5. Excellent verbal and written communication skills.
  6. Ability to represent the company effectively in a variety of settings, with respect to service area, payer sources, etc.
  7. Demonstrated record of strong interpersonal skills.
  8. Competence in basic PC skills required to perform job functions. (Word, Excel, SharePoint, Outlook))
  9. Ability to read, write and effectively communicate in English.

 

PRIMARY RESPONSIBILITIES:

  1. Demonstrate and communicate the core values of BAYADA and The BAYADA Way.
  2. Develop yearly marketing plan and strategic quarterly goals
  3. Interacts with referral sources to facilitate positive long-term relationships to represent BAYADA as the preferred home care provider for complex, chronically ill patients.
  4. Educate and engage prospective clients and their families about home care and prepare them for the transition home, in collaboration with the client’s physician.
  5. Promote effective care coordination with facility,and communicate clinical status and staffing needs to the service office and HMC/VMC to ensure high quality and safe transition to the home care clinical team.
  6. Provide post-transition home follow up telephonically for 2 months to confirm effective care handover and positive outcomes to prevent re-hospitalization of the client the first thirty (30) to sixty (60) days.
  7. Develop and edit quarterly and year to date referral and reports on TCP SharePoint site.
  8. Collaborates with Directors in targeting business development opportunities.
  9. Plans and coordinates joint venture marketing efforts with multiple service offices.
  10. Demonstrate solid performance or exceed performance standards in key job dimensions/attributes
  11. Perform related duties, or as required or requested by Supervisor.

 

OTHER RESPONSIBILITIES:

  1. Coordinates DME, supplies, and in-services needed for discharge.
  2. Represent BAYADA at facility functions, fundraisers, child life events and community activities
  3. Communicates effectively with personnel in all service and support offices.
  4. Possesses knowledge of BAYADA’s nursing policies and attends educational programs.
  5. Demonstrates a team attitude and approach.
  6. Demonstrates professional work behaviors:
    1. Arriving on time and staying late when necessary.
    2. Dressing in accordance with the Transitional Care Program Playbook.
    3. Maintaining proper office demeanor, such as professionalism and confidentiality.

 

BAYADA Offers:

  • Base Salary: $85,000-95,000/ year depending on qualifications
