ESSENTIAL JOB FUNCTIONS AND CORE RESPONSIBILITIES

• Accountable to the unit Manager and/or the Director of the department.
• Adherence to identified nursing standards of practice.
• Annually demonstrates competence in critical behaviors for assigned area.
• Displays professional commitment and supports the unit team by promoting unity and teamwork among coworkers by
maintaining a positive attitude and positive working relationships.
• Responsible for the planning, directing, implementation, and evaluation of each patient. The implementation of nursing
care may be delegated by the RN responsible for the patient to other licensed nursing staff, or may be assigned to
unlicensed staff, subject to any limitations to their licensure, certification, level of competency, and/or regulation. The
RN will direct care provided by the LVN, CNA, nursing student, and/or logistics technician.
• Responsible for understanding and participating in the organization-wide Performance Improvement Program through
orientation, education, departmental and interdepartmental quality control, performance improvement, and quality
planning activities.
• Demonstrates the IRAISE values in all professional and patient care interactions.
• Performs other duties as assigned by Manager / Director / Clinical Lead RN.
• Prioritizes cases to provide service to most urgent case.
• Organizes patient transportation, surgical team coordination, age specific admission evaluation and pre-operative
preparation of patients including pre-operative work-up.
• Utilizing established protocols as applicable, follows appropriate pre- and post-procedure orders based on patient
evaluation.
• Follows up on patients post-procedurally.
• Participates with patient/family and other healthcare providers in the evaluation of patient outcomes.
• Provides coaching and mentoring of staff to improve patient outcomes.
• Coordinates pre and post procedure patient/family education based on the patient’s age/developmental needs, level of
understanding, cultural considerations, barriers, motivators, or limitations.
• Coordinates discharge planning.
• Communicates with patient’s families, medical consultants, medical and hospital staff, outside referring physicians and
medical groups.
• Communicates with insurance company utilization review and authorization personnel.
• Gathers and maintains data and reports as required by government agencies and/or the Quality Review
Department/Hospital Administration.
• Acts as a resource person for nurse coordinators, nursing, and other hospital staff.
• Reviews, interprets, integrates, and analyzes patient clinical findings.
• Effectively communicates finding and recommendations to achieve patient outcomes.
• Identifies the need for and coordinates patient/family conferences as needed.
• Ensures that a physician specific plan of care is in place for patients.
• Proposes alternative treatment options to ensure a cost effective and efficient plan of care.
• Reviews clinical findings and diagnostic reports and correlates medical record findings with patient assessment findings.
• Identifies and creates solutions to remove barriers that impede optimum patient care delivery.
• Provides supportive and therapeutic clinical interventions as indicated.
• Ensures referrals for consultants are completed in a timely manner and follow through as appropriate.
• Identifies the lead physician in complex cases with multiple practitioners.
• Ensures appropriate resource utilization relevant to the financial, regulatory and clinical aspects of care.
• Provides consultation and mediation services to facilitate patient progression towards desired outcomes.
• Collaborates with the entire health care team to identify primary patient/family decision maker and communicates care
plans as indicated.
• Identifies and creates solutions to remove barriers that impede optimal patient care delivery.
• Identifies and records avoidable delays and days.
• Documents and resolves/refer any discrepancy form standards of care issues to responsible individuals.
• Reviews, interprets, integrates, and analyzes patient clinical findings.
• Effectively communicates finding and recommendations to achieve patient outcomes.
• Identifies the need for and coordinates patient/family conferences as needed.
• Ensures that a physician specific plan of care is in place for patients.
• Proposes alternative treatment options to ensure a cost effective and efficient plan of care.
• Reviews clinical findings and diagnostic reports, and correlates medical record findings with patient assessment findings.
• Identifies and creates solutions to remove barriers that impede optimum patient care delivery.
• Identifies potential risk management or quality issues and resolves them through intervention or refers as appropriate.
• Ensures appropriate resource utilization relevant to the financial, regulatory and clinical aspects of care.
• Provides consultation and mediation services to facilitate patient progression towards desired outcomes.
• Collaborates with the entire health care team to identify primary patient/family decision maker and communicates care
plans as indicated.

JOB REQUIREMENTS

Minimum Education: 
Associate’s Degree Nursing

Minimum Skills: 
Possess basic computer skills.


Required Certifications: 
Registered Nurse - RN (CA Board of Registered Nursing)
Advanced Cardiovascular Life Support (ACLS) Healthcare Provider from American Heart Association.
Basic Life Support (BLS) Healthcare Provider from American Heart Association.

Preferred Education: 
Bachelor’s Degree Nursing

Preferred Experience: 
1 year acute care experience.
 Current RN experience in assigned area of Nursing.
 A registered nurse with current RN experience in Cath lab or Interventional Radiology.


Pay Transparency

The hourly rate range for this position is $21.00- $25.95. The hourly rate range for this position is $53.00 - $87.45. POSITION SUMMARY

Responsible for case coordination for the cardiac structural heart procedure patients (TAVR, Watchman, Mitraclip). Coordinates the delivery of patient care from pre-admission to post-discharge. Collaborates with the physicians and entire patient care team in moving the patient efficiently through the continuum of care. Complies with the California Nurse Practice Act, Article 2, Section 2725 of the Business and Professions Code. Assist with coordination of IR related procedures as time permits.
