Hospice Clinical Manager – Riverside Home Care Division

Overview
The Hospice Clinical Manager plays a pivotal role in ensuring the smooth and effective operation of the Agency—day or night. Working under the supervision of Regional Director of Hospice, this leader works steps in for the Administrator and/or Director of Nursing as needed, overseeing clinical functions, staffing, and patient care operations across the Riverside Home Care Division.
In this role, you’ll guide nursing teams, interpret and apply policy, support compliance, and ensure the right resources are in place to deliver exceptional care. You may also function in a hands-on RN capacity—assessing, planning, implementing, and evaluating care for patients across varied age groups and clinical needs.
This position blends leadership, clinical expertise, and critical decision-making while working closely with a multidisciplinary team to deliver services aligned with Riverside Health System’s standards of excellence and strategic goals.
What You Will Do
Lead Clinical Excellence
- Oversee and manage nursing care throughout the Agency, ensuring high-quality patient care supported by appropriate staffing and clinical resources.
- Supervise and delegate the interdisciplinary team members to promote safe, effective, patient-centered care.
Ensure Smooth Daily Operations
- Direct day-to-day operations in the absence of the Regional Director of Hospice.
- Maintain professional, efficient facility operations while supporting regulatory and organizational standards.
Champion Safety & Quality
- Ensure a safe care environment by complying with RHS policies, procedures, and regulatory requirements.
- Investigate nursing errors, patient/visitor incidents, and complaints; provide timely documentation and counseling as needed.
Support Team Collaboration
- Build strong, trusting relationships with the multidisciplinary team.
- Actively participate in team meetings and promote open communication to enhance care coordination.
Develop & Mentor Staff
- Train and educate team members on policies, procedures, best practices, and strategic goals.
- Provide supervisory visits, assist with care planning, interpret policies, and guide problem-solving.
Respond to Patient Needs
- Recognize changes in patient condition and promptly notify physicians or team members.
- Document concerns or quality issues and ensure prompt follow-up.
Oversee Staffing & Resource Allocation
- Adjust staffing based on census and clinical needs to maintain quality care while managing overtime and budget targets.
- Document changes in staffing, patient care needs, and variances in real time.
Qualifications
Education
- Graduate of an accredited Nursing program (Required)
- Bachelor’s degree in Nursing (Preferred)
Experience
- Nursing experience (Required)
- Supervisory experience (Preferred)
Licensure & Certifications
- Registered Nurse (RN) – Virginia Department of Health Professions (Required)
- CPR/BLS Certification – AHA/ARC/ASHI; must be obtained within 30 days of hire (Required)
- Valid driver’s license (Required)
- Must drive a personal vehicle for RHS business 25% or more of the time
