Hourly$51.79 - $77.69The Case Manager applies the process of assessment, planning, implementation, monitoring, evaluation, and coordination of care with the goal of ensuring a safe and sustainable discharge from the inpatient setting to the most appropriate level of post-acute care. By integrating evidence-based clinical standards, preventive guidelines, protocols, and other metrics in development of patient centric treatment plans, the Case Manager promotes quality and efficiency in the delivery of care. The Case Manager also uses the nursing process to provide education and resources to facilitate informed decision-making and timely access to post-acute care. Works in collaboration with the health care team to ensure safe, quality and cost-effective care.
Minimum Qualifications
Minimum of a bachelor’s degree in nursing is required. Master’s degree in nursing preferred
Licensed as a Registered Nurse in the state of Washington required. Current Healthcare Provider CPR certification required. Certification for Case Management (ACM or CCM) preferred.
Minimum of 5 years’ experience in acute care Nursing required. An additional 3 years’ experience in discharge planning or case management experience is preferred.
Strong knowledge of discharge planning, community resources, and payer requirements. Proficiency in EPIC EMR and clinical decision support tools such as MCG and/or InterQual. Knowledge of word processing, spreadsheet, database, and presentation software. Ability to effectively present information and respond to questions from groups of managers, patients, physicians, and payers
