Job Description Summary:
Supervises select care management functions and daily operations.Responsibilities And Duties:
Demonstrates excellence as a case manager and serves as role model for the department. Participates in all meetings specific to job responsibilities (i.e., multidisciplinary rounds, team huddles, and enterprise-wide town hall meetings.) Monitors performance of staff and provides coaching as required. Assures the 90-day onboarding for associates meets the required competencies. Administrative responsibilities for scheduling staff, daily assigning of cases, caseload balancing and assisting the manager (s) with direct supervision of the care management staff and the day-to-day operations/activities of care management. Additionally, the Care Management Clinical Lead will maintain competencies associated /assigned in the Care Manager role (s) and processes. The lead may take caseloads as assigned by manager. Provides input regarding associate performance. Creates work environment where employees can express opinions openly.Minimum Qualifications:
Bachelor's Degree (Required), Master's Degree: Social Work (Required)RN - Registered Nurse - Ohio Board of NursingAdditional Job Description:
MINIMUM QUALIFICATIONS
Bachelor's Degree from accredited school of Nursing (Required)
BLS - Basic Life Support - American Heart Association
RN – current Registered Nurse license in State of Ohio
Minimum 3 years clinical experience as an RN with at least 1 year in Care Management
Demonstrated leadership skills
