Our Cardiology Clinic supports a wide variety of patients with their cardiac needs. This includes preventative care, coronary artery disease, hypertension, heart failure, atrial fibrillation and more. We have a large group of providers that work closely with the RN's, clinical support staff, scheduling staff and more to provide excellent patient care. We are excited to add an individual to our clinic who enjoys being part of a team and has a passion for caring for patients!
What you will do as an RN Care Coordinator:
Coordinates care across the health care continuum within designated sub-specialty population(s)
Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services
Completes pre-visit planning and post-visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visits
Facilitates proactive disease management and preventative health care using evidence-based practice
Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.
Performs other duties as determined by manager and clinic needs
What you will need:
Current Minnesota RN license
BLS certification
Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy
Clinic Description: Employee will float between our Maple Grove and Fridley locations
Location: 14500 99th Ave. N. Maple Grove, and 6401 University Ave. NE Fridley, MN 55432
Schedule: 1.0 FTE, 40 hours per week
