Respiratory Therapist - Case Management
Part-time, first shift
Job Description:
Work collaboratively with respiratory therapy, case management, social services, medical and nursing staff in facilitating the transition of care from hospital to home for COPD and pneumonia patients at high risk for complications and/or readmission. Provide a multi-disciplinary approach while working with current discharge readmission task force team. Oversee high risk COPD and pneumonia patients for the duration of their stay, balancing measures to improve discharge processes to better inform readmission reduction strategies. Administer newly diagnosed COPD/Asthma patients’ education. Partner with case management to manage acute/post-acute respiratory conditions. Coordinate post discharge evaluation of patient outcomes using technology as applicable.
Requirements:
Associate's degree in respiratory therapy or related field
Active license issued by the Kentucky Board of Respiratory Care (KBRC) required.
Registered Respiratory Therapist (RRT) credential through the National Board of Respiratory Care (NBRC) required.
Five years of experience required.
Preferred Qualifications:
Bachelor's degree in respiratory therapy or related field preferred.
Previous experience in discharge planning, case management, or care coordination preferred.
