Position Summary / Career Interest:

The Resource and Referral Coordinator under the direction of the Social Work Case Manager, has responsibility to provide services to assist with resources and referrals identified by Social Work Case Managers based on assessment of need, to patients of all ages and their families receiving care in the clinic. The Resource and Referral Coordinator will work with identified patients to provide linkage to community resources and facilitate connection to related services that may affect optimal treatment outcomes. The primary role for the Resource and Referral Coordinator is to work as an integrated member of the clinic to provide timely linkage and coordination of community resources as established by the healthcare team and directed by the primary Social Work Case Manager.

Responsibilities and Essential Job Functions

  • Partners with the primary SW to identify patients with community resource needs which can be managed by the Resource and Referral role and provides services to these patients under the direction of the primary Social Work Case Manager.
  • Provides ongoing communication of needs and coordinates with primary Social Work Case Manager with any new or changing needs in accordance with the established plan of care.
  • Provides patients/families with timely updates on referrals to various programs to help problem solve barriers to healthcare goals, identifies and provides resources and facilitates referral to community resources.
  • Works collaboratively with patients, families, all members of the healthcare team and community partners to implement a plan for services based on identified needs.
  • Demonstrates knowledge of the available community resources and effectively coordinates linkage to resources based on assessment of need.
  • Communicates timely, relevant and accurate information to all parties involved with patient's care.
  • Participates in performance improvement activities.
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience

  • Bachelor of Social Work in Social Work
  • High School Graduate
  • 0-2 years of experience.


Preferred Education and Experience

  • Healthcare experience


Preferred Licensure and Certification

  • Licensed Social Worker(LBSW) - State Board of Behavioral Sciences State of Kansas Social Work license
