Job Description:

Position: Resident Care Coordinator (CNA)

Location: Gray Birch, Augusta
Schedule: Full-time (40 hours/week)

Shift:  Days 7:30-4:00 M-F (weekend rotation as needed)

What You'll Do:

  • Greets patients and visitors. Answers, screens, and routes telephone calls. Calls codes, pages, and overhead announcements as requested.
  • Performs responsibilities of certification or licensure. Takes responsibility for, or provides guidance to team members on, the handling of complex or novel issues.
  • Assists with processing patient admissions, transfers, and discharges. Assembles and maintains patient records and charts.
  • Performs needed clerical tasks, including copying, faxing, filing, and ordering/obtaining supplies.

What You Bring:

  • An active Maine CNA certification is required
  • Valid Maine driver's license required
  • Previous experience in skilled nursing or long-term care preferred
  • Strong organizational, communication, and leadership skills
  • Commitment to providing compassionate, high-quality resident care
