Rehab Admission Liaison

Location: Main Campus, La Crosse, WI
Department: Inpatient Rehabilitation
Schedule: 0.5 FTE, salaried position | Monday–Friday | 8:00 AM – 4:30 PM (flexible)
No weekends, holidays, or on-call obligations

About the Role

We are seeking a Rehab Admission Liaison to join our inpatient rehabilitation team. This role is critical in ensuring smooth patient transitions and compliance with regulatory requirements. You will act as the primary liaison between referral sources, clinicians, and families, supporting admission coordination and prospective payment system (PPS) responsibilities.

Key Responsibilities

  • Coordinate intake and pre-admission screening, ensuring timely and accurate patient placement.

  • Maintain expertise in rehab admission criteria, CMS regulations, IRF-PAI scoring, and outcome measures.

  • Perform clinical evaluations to determine appropriateness for inpatient rehab.

  • Serve as a consultant on rehabilitation potential, therapy progress, and discharge planning.

  • Partner with external case managers for precertification and payer communication.

  • Educate patients, families, and referral sources on inpatient rehab criteria.

  • Complete PPS reimbursement work and related documentation in EPIC.

  • Collaborate with clinicians during daily consult huddles and maintain strong communication across teams.

Qualifications

Required:

  • Associate degree in a health-related field.

  • Minimum 2 years of clinical experience in an acute or post-acute care setting.

  • Current licensure as RN, PT, or OT in the state(s) of practice.

  • Current Infant–Adult Basic Life Support certification (AHA or Red Cross).

Preferred:

  • Bachelor’s degree or higher in Nursing or related field.

  • 1 year of inpatient rehabilitation experience.

Skills & Competencies

  • Strong clinical background and understanding of rehab criteria.

  • Excellent interpersonal and communication skills.

  • Ability to work collaboratively with clinicians and external partners.

  • High level of professionalism and customer service orientation.
