About the role
The Transitional Aged Care Program (TACP) provides short-term interventions that seek to optimise the functioning and independence of older people after a hospital stay.
TACP is a goal-oriented, time-limited and therapy-focused program to assist people to achieve their maximum potential when recovering from an injury, illness or surgery.
Working as part of a multidisciplinary team to provide discipline appropriate assessment, discharge planning and slow stream therapeutic intervention as well as case management to aged clients in the home.
Appropriate referral to, and consultation with, other relevant health professionals, service providers, and community and support services.
Provide evidence-based nursing treatments with a focus on improved health status, promotion of client independence and adherence to hospital policies and procedures.
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
Current class C Driver's License or above.
Demonstrated ability to work within the interdisciplinary team showing effective communication and interpersonal skills.
Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.
