Role title: Registered Nurse (Year 2-8)

Role type: Max term contract, part time (32 hours per week until July 2026)

Department and location: Transitional Aged Care Program (TACP), St Vincent’s Hospital, Darlinghurst

Salary: $78,346.12 - $104,336.27 (pro rata) + super.

About the role

  • The Transitional Aged Care Program (TACP) provides short-term interventions that seek to optimise the functioning and independence of older people after a hospital stay.

  • TACP is a goal-oriented, time-limited and therapy-focused program to assist people to achieve their maximum potential when recovering from an injury, illness or surgery.

  • Working as part of a multidisciplinary team to provide discipline appropriate assessment, discharge planning and slow stream therapeutic intervention as well as case management to aged clients in the home.

  • Appropriate referral to, and consultation with, other relevant health professionals, service providers, and community and support services.

  • Provide evidence-based nursing treatments with a focus on improved health status, promotion of client independence and adherence to hospital policies and procedures.

About you

  • Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency

  • Current class C Driver's License or above.

  • Demonstrated ability to work within the interdisciplinary team showing effective communication and interpersonal skills.

  • Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.

  • Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.
