Where You Will Work
Support high‑quality, cost‑effective patient care by helping patients transition safely throughout their hospital stay. You will partner with patients, families, physicians, and the interdisciplinary care team to assess needs, coordinate services, plan discharges, and manage utilization in compliance with regulatory standards—ensuring continuity of care across the healthcare continuum.

We’re Looking For
License/Registration/Certification: Registered Nurse License issued by the state in which the team member practices
Level of Education: Bachelor’s Degree in Nursing
Years of clinical nursing experience in - Women's Health or NICU -

What You’ll Do
Conduct comprehensive patient assessments and develop appropriate plans of care with timely interventions
Collaborate and negotiate with patients, families, and multidisciplinary teams to align care goals, preferences, and discharge plans
Provide continuity of care and discharge planning services in compliance with regulatory and accreditation standards
Coordinate smooth patient transfers and transitions across units and care settings
Deliver case management services related to healthcare access, finances, housing, family dynamics, and illness adjustment, utilizing community resources effectively
Initiate internal and external referrals to ensure timely progression of care and transitions
Document discharge planning activities and utilization management interventions accurately and on time
Perform concurrent and retrospective utilization reviews using approved criteria
Partner closely with Social Work and unlicensed support staff to implement safe, effective plans of care
Identify and address barriers to discharge to prevent avoidable delays and expedite care delivery
Collaborate with managers, physicians, medical directors, and physician advisors to support efficient progression of care, accurate patient status, and regulatory compliance
Complete utilization management activities including payer updates, data collection, denial coordination, avoidable delay management, and delivery of CMS regulatory notices within required timeframes
Build productive relationships with community agencies and internal partners, including Ambulatory Care Management and Continuing Health
Serve as an educator and expert resource for medical and hospital staff regarding admission status, acute care criteria, utilization management, care coordination, discharge planning, and regulatory requirements
Provide age‑appropriate care consistent with departmental standards and patient population needs
