Where You Will Work: 

The Atrium Health Transplant Center at 1025 Morehead Medical Drive in Charlotte, NC provides comprehensive, multidisciplinary care for adult patients undergoing evaluation, waitlisting, transplantation, and post‑transplant management. Our program supports adult kidney transplant patients across the continuum of care, including pre‑transplant evaluation and optimization, waitlist management, post‑transplant follow‑up, immunosuppression monitoring, care coordination, and long‑term chronic disease management. The clinic works closely with transplant physicians, surgeons, nurse coordinators, social work, pharmacy, nutrition, and financial services to deliver patient‑centered, evidence‑based transplant care in alignment with OPTN/UNOS and CMS requirements.

We’re Looking For: 

  • Completion of an accredited or approved program in nursing 

  • Registered Nurse license issued by the state in which the teammate practices 

  • Current certification in BLS 

  • BSN required. RN candidates for external hire or internal promotion without BSN must be enrolled in a BSN program upon hire/transfer date and must complete program within 3 years of enrollment in the program  

  • Prior experience in transplant, nephrology, hepatology, or complex chronic disease management

  • Minimum of 3 years of RN experience

  • Strong care coordination, triage, and patient education skills

  • Experience working in an outpatient specialty clinic setting

  • CCTC certification required

What You’ll Do: 

This RN role functions as a lead transplant coordinator within the outpatient transplant program and provides clinical and operational leadership support to the Nurse Manager. In addition to maintaining a working knowledge of transplant coordination workflows, this position serves as a resource and liaison among the transplant coordinator team to promote consistency, efficiency, and best practices. Day‑to‑day responsibilities include supporting ongoing and new program implementation, assisting with quality improvement initiatives, participating in workflow optimization, and contributing to regulatory readiness and compliance activities. The lead coordinator collaborates closely with multidisciplinary team members, supports education and onboarding of staff as appropriate, assists with project coordination and data tracking, and helps ensure alignment with OPTN/UNOS, CMS, and organizational standards. This role balances clinical insight with operational support to advance program goals and enhance patient care delivery. This is a hybrid position with minimum on-site requirements of three days per week.
