St Vincent’s Health Australia is bringing care home. Through our Care at Home portfolio, we are reshaping how care is delivered — extending hospital grade care beyond our walls and into communities, homes and virtual settings that meets people where they are.  

We’re looking for a Registered Nurse to join the Hospital in the Home (HITH) team at St Vincent’s Sydney Care at Home, caring for patients receiving acute hospitalequivalent care in the home. 

You’ll provide exceptional, evidence-based nursing care supporting early discharge and care at home while maintaining strong clinical governance and patient safety. 

About You  

You are a motivated and adaptable Registered Nurse who thrives in autonomous and communitybased models of care. 

You will; 

  • Deliver acute and subacute nursing care to patients in the home or community setting 
  • Conduct comprehensive clinical assessments, care planning and clinical documentation 
  • Administer IV therapies, wound care, antimicrobial therapy and other HITH interventions 
  • Escalate clinical deterioration appropriately in line with HITH escalation pathways 
  • Contribute to quality improvement, safety and accreditation activities 
  • Support a positive patient experience aligned with St Vincent’s values 

You Bring  

  • Current AHPRA registration as a Registered Nurse 
  • Postgraduate nursing experience (acute, medical, surgical or community preferred) 
  • Previous community experience or HITH. 
  • Demonstrated interest or experience in Hospital in the Home, or community nursing 
  • Strong assessment, communication and timemanagement skills 
  • Confidence working independently in the community with appropriate escalation 
  • Current driver’s licence and willingness to travel between patient homes
