BAYADA Home Health Care has immediate opportunities on our Transition Team for RNs & LPNs interested in helping infants and young children during their continued transition from the hospital to home in the Western Pennsylvania area! ​

The Transition Team works in a team-based environment acting as an extension of our HICU® team, delivering quality skilled nursing care to clients at home. As a Transition Team nurse, you have the opportunity to deliver quality care to clients and their family for the initial part of their homecare, while providing extensive training to the family and ongoing care team. Compensation & Schedule:

  • Competitive pay ranges
    • Competitive hourly pay up to $40/hour - final rate determined by experience, clinical skills, and case assignment needs

Qualifications for Transition Team Nurses:

  • Have a current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Have at least one year of verifiable work experience as an RN or LPN, preferably with pediatrics
  • NICU, PICU, ICU, or home care trach/vent experience preferred
  • Achieve a passing score on high-tech Adult and Pediatric simulations.
  • Hold current CPR certification
  • Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
  • Availability to commit to an ongoing, consistent schedule
  • Ability to travel throughout Allegheny, Fayette, Washington, & Westmoreland Counties

Expectations:

  • Willingness to travel (up to 1 hour throughout counties outlined above)
  • Monthly weekend commitments (at least 2 weekend shifts per month)
  • Reliability & flexibility- willingness to meet overall client scheduling needs with other Transition Team members
