Job Description:
Works with patient populations identified as “at-risk” in collaboration with their interdisciplinary care team. Identifies and assists in closure of gaps in care related to preventive care, chronic disease care and regulatory services. Assists in delivery of high quality, efficient, and cost-effective healthcare services. Under general administrative direction, gives patient education related to their condition and planned treatment, promote patient self-management, suggests care plans with providers, manages transitions of care, etc.

- BSN or ADN degree from an accredited school or college of nursing

Care Coordinator Role:
- Current RN license in the state of Minnesota, and Wisconsin upon hire. Ability to obtain RN license in both states within 3 months of hire
Preferred Qualifications:
- Minimum of 2-3 years of clinical nursing experience in hepatology, gastroenterology, and experience with pre-and post-transplant patients.
- Clinical knowledge of liver diseases.
- Experience with patient education: ability to explain disease management strategies, medication adhereance, dietary, and lifestyle modifications.
