Responsible for being a Registered Nurse Case Manager who directs and supervises all Home Health Services personnel involved in patient care within a specific geographic area.  Also responsible for the care of Home Health Services patients and the allocation of agency resources.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Makes initial patient visits, recertification visits, resumption of care visits, Home Health Aide (HHA) and Licensed Practical Nurse (LPN) supervisory routine visits, and discharge visits.
  • Consults with the attending physician in establishing and updating the Plan of Care.
  • Participates in the Home Health Services Interdisciplinary Group meetings as appropriate.
  • Completes OASIS documentation at Start of Care, recertification, resumption of care, transfer, and discharge.
  • Consults with the attending physician in establishing and updating the Plan of Care.
  • Provides quality home nursing care as defined in the RN State Nurse Practice Act while demonstrating fiscal responsibility and maintaining the highest standards of care and ensuring compliance with all agency guidelines and regulatory requirements.
  • Initiates Plan of Care following a comprehensive assessment in accordance with physician orders.
  • Collaborates with other disciplines as indicated to review/revise the Plan of Care, including a discharge plan, based up on the assessment of patient needs.
  • Provides accurate and timely documentation of patient services to reflect the Plan of Care.
  • Assesses and provides patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
  • Participates in the coordination of agency services, appropriately reporting the identified needs for agency Home Health Services Interdisciplinary Group members.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.  Also participates in Home Health Services quality management program.
  • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
  • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
  • Performs other duties as necessary to ensure the success of the System.

SKILLS AND ABILITIES

  • Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing.
  • May be in multiple locations in a work day, including patient homes and Home Health Services offices.
  • Ability to establish and maintain effective working relationships with associates, department heads, administrators, member organization presidents and their leadership teams, and the general public.

MINIMUM QUALIFICATIONS

  • Graduate from an accredited School of Nursing.
  • Currently licensed in good standing to practice as a Registered Professional Nurse in the State of Georgia.
  • Must have no less than six months experience in acute care setting as an RN prior to home care assignment, or experience as required by state regulations.  Over one year of experience as an RN preferred.  Experience in Home Care preferred.
  • Ability to meet Home Health’s conditions of employment regarding health clearance, and provisions of professional references.

SUPERVISORY RESPONSIBILITIES

Supervision of clinical care plan with Home Health Aide (HHA) and/or Licensed Practical Nurse (LPN) as required by regulation and guidelines.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before groups of customers or associates of the organization.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram format.  The ability to deal with problems involving several concrete variables in standardized situations.

This job requires proficiency with the following computer applications:

CERTIFICATES, LICENSES, REGISTRATIONS

  • Must be a registered nurse currently licensed in the State of Georgia with at least one year of nursing experience preferred. A valid driver’s license is required.
  • Current CPR Certification
  • Valid Driver’s License
