What You’ll Do:

  • Care Coordinator Extraordinaire: Design, implement, and evaluate treatment plans by collaborating with your healthcare team, ensuring each patient receives the best care at every stage of their journey.
  • Patient Empowerment: Work with your patients to help them manage their illnesses and navigate the healthcare system, supporting them to achieve optimal health outcomes.
  • Health Home Champion: Provide and manage care for Health Home enrollees, tracking their healthcare outcomes and making adjustments to treatment plans as needed.
  • Team Collaborator: Actively participate in and coordinate patient care with other team members, fostering a holistic approach to treatment and recovery.
  • Evidence-Based Expert: Use evidence-based information to drive care decisions, ensuring that every patient receives high-quality, up-to-date treatment.

What You Need to Bring:

  • Education: BSN (or equivalent combination of ADN and relevant experience).
  • Licensure: Current Missouri RN license.
  • Certification: Current BCLS (Basic Life Support) certification.
  • Experience: A minimum of three years working in a healthcare setting.
  • Communication Skills: Strong written and verbal communication skills—you’ll need to effectively coordinate with patients and the healthcare team.
  • Tech-Savvy: Proficient in Microsoft Word, Excel, and PowerPoint.
  • Language Proficiency: Must be able to read and speak English fluently.

Preferred Qualifications:

  • Community Service Experience: Any experience working in community health is a plus.
  • Multi-language Skills: Ability to speak multiple languages is a bonus!
