Overview of Position:

Job Profile Summary
Plans, provides and/or supervises the nursing care of patients in an efficient and caring manner. Coordinates care planning with other caregivers and with other areas, adheres to universal precautions, and is compliant with infection control policies. Works independently with minimal supervision.
ESSENTIAL DUTIES
1.    Demonstrates competency and effective use of the EPIC documentation system, able to efficiently complete documentation utilizing the Admission/Discharge/Transfer-Arriva Navigators.
2.    Facilitates admission of patients to the nursing unit, completes thorough EPIC admission Navigator documentation, initiating consults related to discharge needs (PT/OT, Speech, Diabetic Educator, Dietitian, ET, etc) initiates care plan and patient education, completed admission assessment if time permits.
3.    All patient teaching elements will be initiated to include introduction to patient room (communication white board, call button, hourly rounding, bedside shift report, patient safety practices, mobility board, 5 Steps to Discharge and CMU Welcome video)
4.    Facilitates discharge of patients, completes thorough EPIC Discharge Navigator documentation, providing education on medications, discharge teaching, completing the AVS and reviewing this with the patient/significant other, and facilitating any nursing home transfers. 
5.    Facilitates the transfer of patients to the nursing unit, completed thorough EPIC Transfer-Arrival Navigator documentation, providing all patient teaching elements as described above.
6.    Provides detailed handoff report to the receiving nurse and ensures needs are addressed prior to leaving the room.
7.    Participates in MDR and /or staff meetings to gain guidance if priority of discharges/admissions or both. Works closely throughout the day with the charge nurse, keeping informed of any changes.
8.    Works in connection with the staff nurse to identify incomplete tasks to facilitate timely discharges. 

QUALIFICATIONS
1.    Current Ohio RN licensure. 
2.    Associates or bachelor’s degree in nursing. 
3.    Minimum of one year of RN experience in a Medical Surgical Nursing unit. 
4.    CPR certification within 30 days of employment.
5.    Ability to read, write and explain or describe moderately complex information to other hospital staff.
6.    Attention to detail.
7.    Ability to communicate well in an oral and written manner, as well as the ability to observe, assess, guide, and evaluate.
8.    Ability to read, calculate and prepare medication, as well as the ability to observe and analyze any resulting reactions and to apply clinical knowledge.
9.    Proficient in assessment skills including auscultation and palpitation.
10.    Demonstrates competence in age-specific patient care.
11.    Positive and professional communication with internal and external customers. 
12.    Critical thinking skills using independent judgement in making decisions.
