LOCATION: Dialysis Center -Spencerport

DEPARTMENT: Renal Dialysis Unit

SCHEDULE: Flexible 8 Hour Day Shifts; Monday to Friday

will be covering both Spencerport and Chili outpatient dialysis centers

Position Summary:

Access, plan, implement and evaluate the nutritional care of Unity Health System patients. 
Key Responsibilities:

  • Works as a member of the multi-disciplinary Nephrology team to maintain/improve the nutritional status of patients by assessing nutritional needs and by planning, implementing and evaluating nutritional care.
  • Communicates formally and informally with Health Care Staff to promote coordination of nutritional care with total patient care.
  • Develops and/or assesses appropriate nutrition education materials for use in patient education.
  • Counsels individuals/families in nutrition principles, adapting instructions to accommodate individual, economic, educational and life style requirements.
  • Obtains and charts an accurate nutritional assessment on chronic dialysis patients within one month of admission to the chronic unit.
  • Reviews and assesses monthly clinical and laboratory data on all chronic patients. Reviews this data with individual patients, provides dietary counseling and documents in the medical record in a timely manner.
  • Completes dietary section of multi-disciplinary care form within two weeks of lab draws.
  • Participates in multidisciplinary patient care conferences within thirty days of admission to the chronic setting and every six months thereafter for each patient.
  • Monitors Urea Reduction Ratio information and performs kinetic modeling as needed; interprets and documents the adequacy results in the medical record.
  • Uses appropriate lines of authority.
  • Actively participates as a member of the Quality Assurance Committee.
  • Remains available to consult on nursing home/acute renal failure admissions as requested.
  • Participates in orientation of new staff and provides ongoing educational in-services.
  • Demonstrates organizational ability and is able to prioritize.
  • Communicates with all departments within the health system in a courteous and respectful manner at all times.
  • Functions independently as well as a team member.

 
Minimum Qualifications:

  • 1 year of clinical nutrition experience as a Registered Dietitian required.
  • Bachelor’s Degree from the Commission on Dietetic Registration (CDR) accredited program required.

 
Required Licensure/Certification Skills: 

Registered Dietitian by the Commission on Dietetic Registration (CDR) required.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
