Referral Coordinator – Health System Partnerships (Contract) @ Midi Health 💻

Midi Health is expanding its health system and provider partnerships and is seeking a Referral Coordinator (contract) to support inbound referrals from partner organizations. In this role, you will engage referred patients, coordinate appointment scheduling, and ensure referrals move efficiently through Midi’s intake workflows.

Working closely with the Commercial team, you will interact with patients daily through Zendesk, phone outreach, and referral management systems to support a seamless patient experience.

The ideal candidate has experience in healthcare operations, patient scheduling or coordination, or referral management and enjoys patient-facing work within structured operational workflows.

Business Impact — Driving Seamless Referral Coordination 📈

You will help ensure referred patients successfully schedule and access care at Midi through three core focus areas:

1. Referral Intake & Coordination

  • Manage inbound referrals from health system and provider partners, ensuring accurate intake and documentation
  • Engage referred patients and assist with scheduling their Midi appointments
  • Ensure referrals move through established workflows efficiently and without delays

2. Patient Navigation & Support

  • Serve as a point of contact for patients navigating care between Midi and its health system and provider partners
  • Answer patient questions about Midi services, referral pathways, and next steps in care
  • Provide clear, supportive communication to ensure a positive patient experience

3. Referral Workflow Management

  • Manage the dedicated Zendesk inbox and voicemail for inbound referral communications
  • Share updates and operational feedback with the Commercial team regarding referral activity and workflow improvements

What you will need to succeed 🌱

  • 2–4 years of experience in care coordination, referral management, patient navigation, or patient support
  • Experience working in a medical practice, healthcare organization, or digital health environment
  • Daily hands-on experience using Zendesk or a similar ticketing platform to manage patient or customer support queues
  • Strong experience communicating with patients via phone and written channels
  • Comfortable managing high volumes of inbound requests and patient communications while maintaining a positive patient experience
  • Ability to follow structured workflows and SOPs while identifying opportunities to improve operational processes
  • Experience working with EHR systems and operational tools
  • Strong communication and cross-functional collaboration skills
  • Comfort working in a fast-paced, growing healthcare organization

The interview process will include 📚

  1. Hiring Manager Interview (30 min)
  2. Final Team Interviews (30min)

This is a remote 1099 contractor role reporting to the Senior Health System Partnerships Lead within the Commercial team. The pay rate is $23hr. No benefits are provided with this opportunity.
