Job Summary:

Manages reporting of quality metrics across Home Health. Assesses and analyzes outcomes data to report out on quarterly metrics and identify opportunities for improvement. Works with the Director of Quality Management and Infection Control to identify focus areas related to quality and provide actionable data.

Position Responsibilities:

Prepare and present regular reports on QAPI initiatives, compliance audits, and other relevant metrics to senior management.

Collaborate with the Director of Quality Management to ensure that all regulatory reporting requirements are met in a timely manner.

Prepare comprehensive responses for Medicare audits, ensuring all documentation and evidence is accurately presented to support compliance.

Works with Director to plan agenda and presentation content for meetings. Aids in the development of quality measures to be presented and approved by committees.

Responsible for overseeing review of Home Health and Hospice Policies and Procedures.

Part of the leadership team tasked with research and development of new post-acute and community-based programs and/or business lines that will reside under the Home Health and Hospice division.

Is an active member on various system councils and system committees to provide input on the development and implementation of new initiatives and processes.

Sits on local interdisciplinary committees as appropriate to coordinate and implement quality and safety initiatives based on outcomes.

Compile, aggregate and assist with trending of quality data in the following areas: patient care, safety, risk management, chart/documentation audits, infection control, outcomes, public reporting, care compare, and customer satisfaction.

Organize and conduct training sessions for staff on QAPI initiatives and compliance requirements.

Develop and maintain training materials, ensuring they are up-to-date with current regulations and best practices.

Participate in on-boarding educational training and education programs based on company needs.

Develops effective education and communication related to Quality management programs.  Acts as a resource for staff.

Work closely with location, regional and corporate leadership to address patient care issues with a goal toward resolution while maintaining confidentiality

Assists in reporting, investigating and tracking safety events.

Maintains an understanding of regulatory and contractual requirements for quality and utilization. 

Supervises Quality, Infection Prevention and Education teams, delegates tasks and projects, monitors productivity and acts as a back-up to departments and needed for PTO, FMLA, etc.

Position Qualifications Required:

Required Experience:

3 years’ experience in a QAPI and/or compliance role required, preferably in a hospice or healthcare setting and 2 years' experience as a Supervisory experience preferred. 

Required Education:

BSN Required. Master’s degree preferred.

Training / Certification / Licensure:

RN licensure. Certification in Health Care Quality required or eligibility to sit for CPHQ Exam within 2 years. 

Annual Salary: $82,033 - $130,988 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
