About the Role

Hopscotch Primary Care is seeking a mission-driven Psychiatric-Mental Health Nurse Practitioner (PMHNP) to support the behavioral health and psychosocial needs of our patients across Western North Carolina. As Hopscotch continues to grow and serve patients with increasingly complex behavioral health needs, this role will play a critical part in expanding access to high-quality, integrated behavioral health care across our clinics.

This role will be embedded in a team-based, primary care environment and focus on psychiatric assessment and diagnosis, medication management, and co-management of patients with behavioral health conditions. The Psychiatric NP will partner closely with primary care providers, care teams, and external behavioral health partners to deliver coordinated, patient-centered care.

This role offers flexibility in care delivery, including a mix of in-person and telehealth visits, and will involve travel to clinic locations across Western North Carolina.

What You'll Do

Specific responsibilities will include, but are not limited to:  

  • Conduct comprehensive psychiatric evaluations and diagnostic assessments
  • Provide psychiatric medication management, including initiation, titration, and monitoring of psychotropic medications
  • Provide counseling via in-person and telehealth visits, with visit frequency and duration guided by patient need and clinical judgment
  • Collaborate with primary care providers to support co-management of patients with behavioral health conditions
  • Partner with care teams to align on behavioral health care plans
  • Provide consultation and clinical guidance to care teams on behavioral health diagnoses and treatment strategies
  • Support patients with serious mental illness and complex behavioral health needs, including care transitions and stabilization
  • Coordinate care with external psychiatric providers and specialty behavioral health resources
  • Participate in clinical team meetings to align on patient care plans
  • Accurate and timely documentation of patient encounters and care coordination activities
  • Contribute to the ongoing development and refinement of Hopscotch’s Behavioral Health Program

About You

You would be a great fit for this role if you:

  • Hold an active Psychiatric Mental Health Nurse Practitioner (PMHNP) certification
  • Are licensed or eligible for licensure in the state of North Carolina
  • Experience utilizing electronic medical record systems; specific experience with athena is a plus, but not required
  • Have experience providing psychiatric care in outpatient or integrated care settings
  • Are comfortable managing patients with complex behavioral health needs, including serious mental illness and substance use disorders
  • Have strong knowledge of psychopharmacology and evidence-based treatment approaches
  • Communicate clearly and collaboratively with primary care providers and care teams
  • Thrive in a fast-paced, evolving environment and are comfortable helping build structure where needed
  • Are willing and able to travel to clinic locations in Western North Carolina
