Schedule: M-T, 8:30 am - 12:30 pm
SUMMARY:
The PROS Clinician is a member of the PROS clinical team whose primary function is to provide assessment, outreach recovery planning and actively support recovery goals of PROS Participants. As such, the PROS Specialist assists PROS participants with skill development and elimination of barriers to recovery. Works in collaboration with therapeutic treatment team to improve lives of people with serious mental health disabilities. The PROS Clinician works in partnership with LPHA’s/clinical staff on assigned caseload where required for participants enrolled in PROS with Clinic.
RESPONSIBILITIES:
Participates as a contributing full member of the multidisciplinary treatment team. Works in collaboration with Team Leader/LPHA on clinical needs of assigned caseload.
Develop/facilitate class curriculum for skill building classes.
Provide CRS, IR, Clinical Symptom Monitoring and ORS interventions as outlined in PROS participant IRP. Symptom monitoring services will be reviewed and co-signed by MD/RN
Assist PROS participants with goal setting and skill building through group facilitation and individual contact. Provides Outreach and Engagement.
Individual Recovery Plans – Incorp0orate individual strengths, aspirations, barriers from assessments and reflect participants’’ full input. Participants on assigned caseload have clearly defined recovery goals which are stated in their own words.
Facilitates assessments and individual recovery plans (IRP’s) development with PROS participants.
Provides vocational assessments, psychiatric rehabilitation assessments. Participants on assigned caseload have clearly defined recovery goals (i.e., employment).
Documentation is completed within designated timeframes. Documents are forwarded to Program Supervisor/LPHA in a timely manner.
Maintains minimum productivity expectations for assigned caseload.
Uses recovery and person centered approaches, including person first language when working with consumers and families. Provides ongoing support for participants for goal acquisition (i.e., employment).
Demonstrates safe driving practices in accordance with Rochester Regional Health’s driver policy.
Participates fully in Quality Improvement activities within the PROS program and department.
Collaborates and communicates with primary care providers and multidisciplinary team as appropriate and documents contacts in participant’s record.
Participates in and incorporates professional supervision and education into practice. Promotes and demonstrates good communication and team work through participation in daily staff meetings, case conferences and team meetings.
Participates functionally as a member of an interdisciplinary team in coordination and delivery of care. Demonstrates knowledge of and provides linkage with community and natural supports. Communicates with relevant internal and external care providers to complete ongoing assessment to coordinate care.

PREFERRED QUALIFICATIONS:
Bachelor’s degree in human services area or related field, or equivalent combination of education and experience.
Two years of behavioral health experience.
Strong communication skills.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
