Job Summary: The Veterans Health Administration Care Coordination Home Telehealth Program (CCHT) service uses telecommunication technologies to expand care beyond institutions. The program collaborates with primary care and mental health providers to enhance coordination and managing acute, chronic, and psychiatric conditions. The objective of the program is to enhance disease management, access, satisfaction, and outcomes while reducing resource use and healthcare costs.

Major Duties:

  • Program Support Assistant(PSA) will serve as administrative support for the Care Coordination and Home Telehealth Program (CCHT). Duties are varied, non-routine and consist of handling a variety of administrative functions necessary to ensure the smooth operation of the office. Duties range from; correspondence and information control, tracking equipment, supplies and services, and tracking data of program patients. Problem solving will require the PSA to make independent decisions about a variety of complex equipment matters and the maturity to interact with the CCHT programs patient population. The PSA will serve as a point of contact for patients, medical center staff, VISN staff, community agencies, and various others who conduct business with the office. They will be responsible for collecting, compiling and/or tracking of statistical information in support of program operations and to access status or outcomes in relation to established performance measures, goals, and objectives; receives and processes documents and reports in accordance with established procedures. Maintains the privacy, confidentiality and security of the patients' medical records and employee data, establishing protocols for incoming data, organizing and maintaining patient data as well as the retrieval of data. Tracks and trends issues related to program effectiveness and role implementation. Utilizes statistical data to compile a monthly workload report for all program staff and administrative supervisors. Attain and maintain computer skills (e.g., word processing, file management, VISTA/CPRS program, EXCEL spreadsheets, PowerPoint) and communication tools such as MS Outlook. Will accurately collect, enter and manage data, including both patient data and program data. Additional duties as assigned. Work Schedule: 0730-1600 M-F Virtual: This is not a virtual position. Qualifications: To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 05/12/2026. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-6. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-6 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: but are not limited to: administratively managing the complex day to day operations of a program or service; advanced use and application of computer software packages such as Excel, PowerPoint and Microsoft Word; composition of reports, audits, memos, action items, agendas, and meeting minutes; tracking, ordering, and distribution of supplies and equipment; assisting with orientation of new employees to include preparation of training materials; scheduling of training rotations; assuring orientation documents are completed and in employee's competency folders; and tracking and monitoring of Performance Improvement activities. Physical Requirements: The work is primarily sedentary, although physical effort is also required in this position. Typically, the employee may sit comfortably to do the work. However, there is walking long distances while pushing supply carts/dollies. Also standing, bending, and carrying/lifting/loading/unloading of items up to 25 pounds such as equipment and supplies, is required in this position.
