Program Coordinator – Housing Navigation Program – Weill Cornell / Lower Manhattan Hospital
As a Program Coordinator for patients in our Lower Manhattan Housing Navigation Program in the Division of Community and Population Health, you’ll be responsible for the overall coordination, management, and execution of our initiatives aimed at addressing the unique healthcare needs of unhoused individuals. Working in collaboration with a multidisciplinary team, you’ll design, implement, and oversee programs and services tailored to meet the specific challenges faced by this vulnerable population. This role will oversee the program’s day-to-day operations by frequent, on-going coordination with hospital staff, Department of Homeless Services, NYC Office of the Medical Director, homeless coalition agencies, and various non-profits. Additionally, this position will collect and analyze data to evaluate the effectiveness of the program, while creating methods to ensure monthly data reports are readily available and visible for other staff to access. This program coordinator will develop processes and monitor program budgets, ensuring financial resources are optimally allocated. Lastly, this role may have limited patient interaction as necessary.
