Job Summary:

Provides care necessary for implementing the therapeutic plan of the physician. Coordinates clinic visits, maintains responsibility for all aspects of outpatient care, and performs clinical research. Assists with inpatient needs. Serves as a patient advocate in the medical care system serving as confidante, advisor, and liaison between patients and physicians.

Job Duties:

  • Coordinates admissions, evaluation, and treatment of patients to the cystic fibrosis and pulmonary disease treatment program.
  • Conducts the intake interview and participates in the initial evaluation and treatment planning.
  • Reviews research and treatment protocol requirements prior to patient's clinic visits to coordinate efforts of appropriate health care team members.
  • Ensures all laboratory results and radiologic results are available prior to patient's visit.
  • Arranges and coordinates the various studies, tests, and consultations as needed throughout the treatment for outpatients and assists with care coordination for inpatients.
  • Provides patients and families with the results of diagnostic tests at time of appointment or via phone.
  • Provides ongoing education to patients and families regarding the disease process directly and through educational resources.
  • In conjunction with the social worker, assists the patients and families with socioeconomic problems and referral to appropriate agencies.
  • Coordinates the health care team meetings regarding the evaluation of patients' care, evaluation of the program and the creation and implementation of recommendations for improvement of the program.
  • Provides ongoing training for the support staff through development of inservice teaching programs about cystic fibrosis.
  • Participates in the budget development and monitoring of allocations of funds.
  • Functions as a liaison to the community by providing information and education for school nurses, home health nurses, teachers, public health nurses, employers, and other agencies about the disease process.
  • Serves as an advocate for children and adults with cystic fibrosis.
  • Coordinates and conducts quality studies applicable to the program.
  • Participates in the research of new program areas for improved patient services and coordinates clinical research projects.
  • Identifies personal learning needs and keeps current in all aspects of cystic fibrosis through reading literature and attending conferences.
  • Collaborates with pharmacists and insurance companies to complete prior authorization for medications and equipment essential for the patients.

  • Education: Graduate from Specialty Training Program
  • Experience: Healthcare - Minimum of 5 years
