Private Duty RN Case Manager (Part-Time)


St. Joseph County & Elkhart County
Monday–Friday | 5 Hours Per Day (Part-Time)

What You’ll Do

As a Private Duty RN Case Manager, you will:

  • Assess clients to determine eligibility and appropriateness for private duty home care

  • Complete admissions and develop individualized plans of care

  • Collaborate with physicians, caregivers, and referral sources to ensure seamless care transitions

  • Coordinate services and resources to support continuity of care

  • Partner with schedulers to ensure appropriate caregiver placement

  • Supervise RNs, LPNs, and Home Health Aides to ensure quality care and accurate documentation

  • Review clinical documentation to ensure alignment with the plan of care

What You’ll Need

  • Graduate of an accredited Registered Nursing program

  • Active RN license in the state

  • 2+ years of clinical or home health experience preferred

  • Strong communication and organizational skills

  • Commitment to high-quality, patient-centered care

  • Ability to work collaboratively within an interdisciplinary team

  • Two weeks of full-time availability required for orientation
