Conviva Senior Primary Care, a subsidiary of Humana Inc., is looking for a full-time, primary care physician for our Gateway center in Jacksonville, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have an unique, value-based care model that supports our senior patients' primary care needs.
As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director. This role will be onsite and in-person at the Conviva Gateway center.
Responsibilities include:
- Evaluate and treat center patients in accordance with standards of care.
- Follow level of medical care and quality for patients and monitors care using available data and chart reviews.
- Assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
- Act as an active participant and key source of medical expertise with the care team through daily huddles.
- Help Regional Medical Director and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
- Complete all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.
A day in the life of a Conviva physician
- Monday-Friday 8:00am-5:00pm
- Join an experienced team with an established patient panel
- Have time with your patients - appointments are typically between 20 and 40 minutes in length
- Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care
Required Qualifications
- Doctor of Medicine or Osteopathic Medicine
- Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
- Valid, unrestricted license in the state of desired employment (or willing to obtain)
- Current, unrestricted DEA license

Preferred Qualifications:
- New resident physicians will be considered
- Minimum of two+ years of experience
- 1+ years of experience managing Medicare Advantage panel of patients
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
- Bilingual in English and Spanish is preferred (not required)

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$219,400 - $306,900 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
