Job Summary: The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) GS-6 functions as a member of the health care team by demonstrating an exceptional degree of practical nursing knowledge and skills necessary for self-direction in carrying out complex practical nursing care assignments within the CBOCs and Specialty Clinics for a Veteran population with a variety of health care needs.

The panel population is multi-faceted, including geriatric patients, patients from all war-time eras, female patients, and patients with complex psychiatric needs. The LPN completes assigned duties and responsibilities that involve performing non-standard and specialized practical nursing activities requiring broad nursing knowledge, experience, and skills to address a wide range of care needs. In addition to performing the duties of a GS-6 PACT LPN, the incumbent demonstrates self-direction in performing an extensive range of nursing care according to established policies and procedures, which includes an expanded scope of practice. The LPN has complex responsibilities at the level of a GS-6, including nurse health maintenance visits, panel management, data analysis, and performance/quality improvement roles. These responsibilities also include being technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Duties include, but are not limited to: Acts promptly in emergency situations Calls for assistance and initiates first aid, CPR, oxygen administration, and assists with defibrillation; Interviews patients, completes clinical reminders and provides patient teaching with documentation in the Computerized Patient Medical Record (CPRS) Takes vital signs and performs other nursing procedures as required, i.e. dressing changes, inserting foley catheters, suctioning tracheotomy tubes, administering breathing treatments in accordance with VA policies and procedures Treats the patient with dignity, as an individual with compassion, respect reasonable protection from harm and appropriate privacy Administers medication correctly and verbalizes classification and side effects accurately Performs exit interviews with documentation and provides patient teaching regarding treatments and medications and documents in CPRS Observes condition of patient and reports information to RN staff;-Accurately performs EKGs and venipunctures as ordered in accordance with VA policies and procedures Prepares patient for examination Follows instructions in the use and cleaning of specialized technical equipment Recognizes and reports equipment that is broken or malfunctioning Provides the highest quality care and services to our customers, which includes patients, family members, medical center employees, and general public by treating them with courtesy, dignity, respect and maintaining confidentiality Performs duties as a team member willingly accepting all assignments. Demonstrates flexibility in performing tasks assigned. Work Schedule: Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions. Licensure: Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English. Grade Determinations: Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.(b) Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.(c) Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies/procedures.(d) Ability to observe, identify and respond to the patient's needs for [care, including] medication, equipment-assisted care and [patient/family education. In organizing and delivering care, the LPN/LVN recognizes and] considers emotional, cultural, spiritual, socio-economic, and age-related factors.(e) Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures.(f) Knowledge and ability to recognize urgent or emergent patient care situations,(g) Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.(h) Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills.
