Population Health Analytics Manager

Job Summary

The Population Health Analytics Manager leads analytics that support population level health management, care coordination, chronic disease programs, health equity initiatives, and operational population health workflows. This role enables clinical and care management teams through accurate patient identification, data driven outreach, and measurable outcomes across ambulatory, inpatient, and community settings.
The Pop Health Manager ensures analytics effectively support frontline workflows, regulatory requirements, and population health strategy.

Essential Functions
  • Lead analytics supporting chronic disease management, high risk populations, and outcome reporting
  • Support care coordination and transitions of care, including TCM workflows, RN outreach tools, ED outreach, and readmission monitoring
  • Deliver and optimize gaps in care dashboards, patient outreach tools, and campaign outcome reporting
  • Oversee patient identification, stratification, and propensity weighted models to support population programs
  • Enable workflow fidelity monitoring to assess effectiveness of population health interventions
  • Support health equity and SDOH analytics, including community referrals, MI 211 integration, and response workflows
  • Deliver population health assessments, regulatory reporting, and compliance documentation 
  • Support interoperability initiatives, including Pop Health queries, MiHIN, and patient reported data
  • Enable digital population health tools such as MyChart use cases, Care Companion, remote patient monitoring, and patient reported outcomes
  • Partner with analytics, engineering, and clinical teams to ensure reliable, user centered reporting

Qualifications
  • Preferred Bachelor's Degree or equivalent health care management, business, finance, health services research, computer science, mathematics, statistics, informatics, biostatistics, nursing or other related area of health care focus
  • Preferred Master's Degree or equivalent related field
  • 3 years of relevant experience Supervisory, management or project leadership Required
  • 5 years of relevant experience healthcare, business, information services, or data analytics Preferred
  • 3 years of relevant experience Proven expertise with producing reports for multiple audiences including the board of directors, administration, physician and nursing leadership. Preferred
  • 3 years of relevant experience Data extraction from clinical information systems. Preferred
  • 3 years of relevant experience managing teams with disparate roles and focuses, and in directing the work of others Preferred
  • Experience with Epic, MS SQL and other reporting tools Required
