The Clinical Expeditor Advanced Practice Provider (APP) is an advanced clinical operations provider embedded within the CCLC. The APP supports real-time capacity management, patient throughput, and transitions of care across inpatient units, the ED, and perioperative environments. The APP partners with inpatient clinicians, ED providers, perioperative teams, imaging/diagnostics, outpatient departments, Case Management (CM)/Social Work (SW), the Transfer Center, and Logistics Operations Supervisors. Within scope, the APP performs targeted clinical assessments, initiates or modifies orders, expedites diagnostic and consult workflows, reviews select transfer requests, and identifies opportunities to shift appropriate hospital-based services to outpatient settings.


Essential Duties and Responsibilities
  • ADVANCED CLINICAL EXPEDITING & DECISION SUPPORT
    • Conduct focused assessments to address delays in discharge, transfer, or level-of-care change.
    • Initiate or modify orders including labs, imaging, therapy, medication adjustments, and consult prompts.
    • Assist providers with early decision-making for ED boarding patients, ICU/Step-down candidates, and medically complex patients.
    • Identify factors contributing to avoidable days and escalate appropriately.
  • CLINICAL REVIEW FOR TRANSFERS (LOGISTICS COLLABORATION)
    • Collaborate with the Transfer Center, Logistics Operations Supervisors, Logistics leadership, and CM/SW.
    • Review select transfer requests to assess acuity, stability, and readiness.
    • Support appropriate, timely acceptance and prioritization based on capacity and clinical criteria.
  • IMAGING, DIAGNOSTICS & PROCEDURAL NAVIGATION
    • Collaborate with Imaging, Lab, Pathology, and procedural departments to expedite critical studies and results.
    • Review pending diagnostics to clarify sequencing and minimize delays.
    • Identify diagnostics that can transition safely to outpatient settings.
  • ED & PERIOPERATIVE FLOW SUPPORT
    • Provide advanced clinical insight to support ED-to-inpatient flow and reduce boarding.
    • Reassess ED patients to clarify readiness or remaining barriers.
    • Partner with perioperative and PACU teams to facilitate post-op progression.
  • OUTPATIENT COORDINATION & SHIFTING INPATIENT-ORDERED SERVICES
    • Identify services appropriate for outpatient transition (imaging, infusion, wound care, therapies, labs).
    • Coordinate with outpatient scheduling to ensure safe and timely follow-up.
    • Reinforce pathways that reduce unnecessary inpatient utilization.
  • TRANSITION-OF-CARE & COMPLEX DISCHARGE SUPPORT
    • Collaborate with CM/SW to address complex discharges and community resource needs.
    • Participate in complex care rounds to proactively identify barriers.
  • COMMAND CENTER LOGISTICS INTEGRATION & REAL-TIME SUPPORT
    • Maintain presence in the CCLC to use dashboards, analytics, and predictive tools.
    • Provide clinical recommendations during huddles, surge events, and Tier activations.
    • Support Logistics Operations Supervisors in real-time prioritization.
  • WORKFLOW TESTING, INNOVATION & SPREAD
    • Lead or support rapid-cycle testing of new workflows.
    • Collect feedback and outcome data to refine processes.
    • Support transition of validated workflows to operational owners.

Qualifications
  • Master's Degree in Physician Assistant Studies - required
  • 4-6 years Clinical Experience - required
  • Clinical expertise (High proficiency)
  • Communication and professionalism (High proficiency)
  • Ability to problem-solve as it relates to patient flow, Bed Management and Capacity Management (High proficiency)
  • Utilization and data entry into EMR and technology (High proficiency)
  • Ability to interpret real-time data to drive decision-making (High proficiency)
  • Ability to work within and with large teams (High proficiency)
  • PA - Physician Assistant NYS Licensure Upon Hire - required
