Job Summary


Vanderbilt Health is hiring a Physical Therapist Assistant (PTA) to provide home health physical therapy services throughout Nashville, TN, and surrounding Middle Tennessee communities. Responsibilities – Physical Therapist Assistant (PTA), Home Health

  • Provide home health physical therapy treatments in patients’ homes

  • Carry out individualized treatment plans developed by supervising Physical Therapists

  • Promote mobility, functional independence, and home safety

  • Educate patients and caregivers on exercises and safe mobility strategies

  • Document care accurately and timely in EMR systems

  • Collaborate with Physical Therapists, RNs, physicians, and other interdisciplinary care team members

  • Travel throughout the assigned home health service area

Qualifications – Physical Therapist Assistant (PTA)

Required

  • Active Tennessee Physical Therapist Assistant (PTA) license

  • Minimum of one year of PTA experience (home health experience not required)

  • Strong communication and patient‑care skills

  • Ability to work independently in the home setting

  • Valid driver’s license, reliable transportation, and ability to maintain a satisfactory driving record

Preferred

  • Experience with EMR systems

  • Home health or outpatient rehabilitation experience

  • Strong understanding of functional mobility and rehabilitation techniques

Technical Capabilities:

  • Rehab PTA/COTA Patient Documentation (Novice): Demonstrates sufficient proficiency in communicating functional impact of assessment findings, treatment interventions, treatment planning diagnosis, and disease process. Considers the communication needs/level of understanding of audience and may share appropriate information/literature.

  • Treatment Implementation (Fundamental Awareness): Demonstrates proficiency in providing treatment consistent with physician prescription and established plan of care. Able to modify treatment plan as needed to demonstrate provision of skilled therapy services including progression of treatment throughout course of care, and revision of treatment interventions of goals not met. May supervise and direct certified athletic trainers (PT only), physical therapist / occupational therapist assistants, technicians and volunteers and students. Selects and follows correct protocol for care and utilize evidence based treatments.

  • Rehabilitation Patient Education (Fundamental Awareness): Demonstrates proficiency in communicating functional impact of assessment findings, treatment interventions, diagnosis , and disease process. Able to assist the therapist in planning for the discharge needs of the patient. Will adapt communication to needs/level of understanding of audience and share appropriate information/literature. May participate in providing educational experiences and professional development activities.
