Pharmacy Care Coordinator
Verifies patient eligibility and coverage for specialty medications and stays current on payer requirements.
Supports prior authorizations, obtains overrides, and prevents gaps in therapy by maintaining accurate insurance and renewal information.
Triages clinical or insurance-related questions to pharmacists and documents all care activities in the EHR.
Serves as the main contact for patients, providing compassionate support and coordinating specialty medication needs.
Communicates effectively with providers and care teams regarding coverage changes, requirements, and patient needs.
Assists patients with enrollment in financial assistance programs and gathers needed documentation.
Uses technology to manage refills, delivery coordination, outreach tasks, and timely claim processing.
Follows organizational policies, safety standards, and accreditation requirements, participating in needed training and preparation.
Requirements:
Must have a high school diploma or equivalent. Registration with Kentucky Board of Pharmacy upon hire date.
