Job Summary: This Pharmacist serves a broad and expansive role throughout the entire Phoenix VA Medical Center in the coordination and delivery of the Veteran Access and Accountability Act prescription benefit program. Performs the duties of a clinical pharmacist in all phases of assigned areas including outpatient and any assigned clinical activities.

Major Duties:

  • 30 Day Hiring Model The Department of Veterans Affairs is implementing a streamlined 30-Day Hiring Model designed to accelerate hiring while maintaining the highest standards of merit-based selection. This improved process reduces traditional hiring timelines by integrating early coordination, faster assessments, and efficient pre-employment actions. Major Duties include but are not limited to: Administrative/Supervision: Develops and establishes policies and procedures defining operational requirements related to Outpatient Pharmacy and the Community Care prescription program. Assists Outpatient Pharmacy Supervisor with supervising day-to-day operations of Outpatient Pharmacy, esp. Community Care operations. Assesses workload shifts throughout the day, identifying backlogs and making staffing adjustments as needed to meet organizational goals. Collaborates with Outpatient Pharmacy Supervisor and other Pharmacy Leadership for control of all medications, medical supplies, and controlled drugs issued to outpatients. Responsible for performance improvement programs in the Outpatient Pharmacy for technicians including data collection, peer reviews and accuracy screening. Ability to communicate orally and in writing to persuade and influence clinical management and decisions. Program Management: Functions as a Program Manager for the Pharmacy Care in the Community (CITC), including planning, directing, controlling, and coordinating comprehensive distributive outpatient pharmacy services. Assures patient communication is completed concerning prescriptions received from unauthorized care and prescriptions are handled appropriately. Sustains good workflow patterns to maintain desired wait times while ensuring accurate dispensing and record keeping. Works with other pharmacy managers and staff to identify areas of duplication of efforts and develop solutions to streamline processes, reduce waste, save costs, and improve patient care. May serve as a pharmacy service representative on assigned committees and subcommittees. Clinical: Promotes a patient-focused, team-based multi-disciplinary model of pharmaceutical care to promote drug prescribing, medication monitoring and drug laboratory review that assures positive outcomes from medication use. Works to develop and maintain a system that aids pharmacists in identifying actual or potential drug related problems at the front end of the order thereby eliminating duplication of efforts, reducing medication errors, and increasing patient satisfaction. Ensures staff understand how to report actual and/or potential medication errors or adverse events and participates in quality improvement initiatives to promote patient safety. Recommends formulary alternatives and completes non-formulary consults as assigned. Implements P&T Committee policies including therapeutic interchanges and automatic substitutions to include appropriate patient and provider education. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT) Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Pharmacist-Outpatient Community Care Facility Program Manager, GS-13 At least 1 year of experience equivalent to the next lower grade (GS-12) which demonstrates the Knowledge, Skills, Abilities (KSAs) and other characteristics related to the duties of the position to be filled. KSAs described at the next lower grade level include: knowledge of professional pharmacy practice, ability to communicate orally and in writing to both patients and health care staff, knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security, and skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. In addition to the experience mentioned above, the candidate must also demonstrate all of the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Physical Requirements: The work involves long periods of standing and moving about the work unit. The work requires regular and recurring bending, lifting, stooping, stretching, lifting. The incumbent must be a mature, sensible individual capable of working effectively in stressful situations, demonstrating cheerfulness, compassion, courtesy, and concern for staff and patients. Must possess math skills to convert medications to the correct dosing. An ability to perform tasks requiring rapid mental and muscular coordination functions simultaneously is necessary. Incumbent must be physically capable of operating various types of equipment used for the job. Incumbent must possess emotional and mental stability. Must be able to work closely with others.
