The Pharmacist Specialty Infusion works in the HomeStar Pharmacy as a designated provider of pharmacy care and works with other members in healthcare to assure the delivery of high quality products and services related to medications, products, equipment and services, and that such activity is done in accordance with the physician’s orders.
JOB DUTIES AND RESPONSIBILITIES:
- Demonstrates technical competence relating to pharmaceuticals in the setup and maintenance of specialty medications and intravenous products and therapy services for outpatients and home patients.
- Sets up the dispensing of and facilitates the use of administration devices and IV access catheters in accordance with policy and procedure.
- Identifies patient, physician and referral source needs and assists in promoting infusion and specialty pharmacy services.
- Compounds and dispenses medications following prescriptions issued by physicians or other authorized practitioners.
- Ensures new patients and new orders on existing patients are fully assessed for appropriateness of therapy, high risk medications, allergies, adverse indications or interactions, and other potentially adverse outcomes.
- Effectively communicates with multiple professional disciplines, patients and third party payers as required.
- Serves as a resource for issues related to sterile product compounding. Provides assessment and feedback regarding personnel skills in the preparation of compounded sterile products for the purpose of employee performance evaluation.
- Works closely with Pharmacy Technicians, Compounding/IV Technicians, and other pharmacy staff. Assists the Homestar RX and Infusion Manager in training, supervising and developing the Homestar team.
- Sanitizes IV room, equipment and hood.
- Takes inventory and maintains adequate supply of drugs, supplies and other pharmaceutical stock. Removes out-of-date items from the inventory.
- Provides information on potential drug interactions, potential adverse drug reactions, storage and use requirements for prescription medications.
- Participates in quality improvement activities, medication error review, adverse drug reaction review and medication use evaluations as they pertain to compounded sterile products.
- Acts as a consultant to nursing groups and health care practitioners on matters pertaining to pharmacy.
PHYSICAL AND SENSORY REQUIREMENTS:
Sitting for up to eight hours per day and up to three consecutive hours. Standing for up to eight hours per day and up to two consecutive hours. Walking for up to eight hours per day and up to 30 consecutive minutes. Continuously fingering; frequently handling, firm grasping, twisting and turning. Frequently reaching above shoulder level; frequently looking up. Occasionally lifting up to 20 pounds and carrying up to 35 pounds and pushing up to 200 pounds. Occasionally stooping, bending, squatting, crouching and kneeling. Rarely crawling and climbing. Hearing as it relates to normal conversation, high and low frequency. Seeing as it relates to general vision, near vision, far vision, depth perception, peripheral vision, color vision and monotony.
EDUCATION: Must be a pharmacist licensed in Pennsylvania.
TRAINING AND EXPERIENCE: Two years previous infusion or specialty pharmacy experience preferred.
