Job Summary: About the Position: This position is located at Naval Medical Center with the Directorate of Clinical Support, Pharmacy Division in San Diego, California. This is a Direct Hire Solicitation.

Major Duties:

  • Serve as a Clinical Pharmacist Practitioner facilitating pharmaceutical care services and direct patient care programs within assigned service(s).
  • Ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulated standards.
  • Maintain collaborative and comprehensive medication therapy management clinics and services in designated areas of practice, under a scope of practice.
  • Deliver direct care to patients though a variety of modalities and ensure documentation for encounters and clinical decisions in the electronic health records.
  • Communicate effectively and professionally with patients, and other members of the healthcare team with the goal to educate, promote patient safety, and practice advancement.
  • Review electronic medical records to monitor for drug therapy, including evaluation for adverse drug reactions, drug-drug and drug food interactions, disease due to iatrogenic cause, medication therapy duplication, or unnecessary polypharmacy.
  • Identify high-risk patients and implement interventions to improve quality and safety.
  • Perform drug usage evaluations, quality assurance/ performance improvement activities as required.

Basic Requirement for Pharmacist: Education: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Exemption: You are exempted from the basic requirement above if you are a current federal employee occupying a position in the 0660 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status. Licensure: A current, active, valid, and unrestricted license to practice pharmacy in a State, a territory of the United States, or the District of Columbia. Medical Requirement: Must be able to distinguish basic colors. Specialized Experience Required: Must have at least one or more full years of professional pharmacy experience which includes dispensing medication, compounding prescribed drugs/pharmaceuticals, and counseling patients regarding their prescribed medication. This definition of experience is typical of work performed at the next lower grade of GS-11 in the federal service.
