JOB DESCRIPTION:
The Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. They apply specialized knowledge of pharmacotherapy to direct patient care, teaching, consultation and research.
The pharmacist clinician will be responsible for providing care for patients in the surgical intensive care unit at the University of Vermont Medical Center. They will round daily with the surgical intensive care unit teams to optimize therapy and manage medication related problems to promote patient care. In addition, the clinician will provide education in various areas including residency programs, students, and UVMH staff. The clinician will also work collaboratively with other disciplines to create or improve clinical guidelines, policies, or workflows.
Provides clinical leadership in area of expertise including policy development and program oversight responsibilities.
EDUCATION:
Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience. PGY2 in Critical Care preferred. Licensed pharmacist in the State of Vermont.
EXPERIENCE:
3 years of advanced practice experience in their area of expertise or equivalent experience.
