Job Summary:
Responsible for emergency telephone management of poisoning exposures and questions handled by the Poison Center. Guides treatment advice for both the public and health care providers. Manages beneficial relationships with individuals and departments inside and outside of the organization. Accurately questions to aid in decision making responses regarding toxicology recommendations. Evaluates, plans and implements effective patient care plans within the guidelines of the standards of pharmacy practice and operational guidelines.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Utilizes the triage and treatment process for the assessment, planning, intervention, and evaluation in all patient encounters. Uses knowledge of medications in instructing patients about medication therapy.
- Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention.
- Triages/screens telephone call related to patient care. Obtains appropriate information with attention to detail and accuracy (i.e. using direct symptom related questions – keeping patient focused.) Makes assessment utilizing, theory and judgment, and patient information. Implements appropriate care using toxicology guidelines.
- Establishes working relationships, teaching protocols for patient education, family counseling and general public information.
- Takes a leadership role in the facilitation of department functions including mentoring new staff and ensuring policies and procedures are followed. Leads and participates in site process and policy improvement teams.
- Obtains and documents a complete and accurate history while demonstrating appropriate assessment and triage skills in a timely manner. Demonstrates exceptional communication skills providing a calm, reassuring and intelligent interaction with the public and health care professionals.
- Utilization of advancing technology with live chat and texting to assist callers with poison information or exposures.
- Uses knowledge of poison prevention techniques and interprets subtle indications while formulating accurate questions to aid in decision making responses regarding toxicology recommendations.
- Creates an environment that leads to a positive patient care experience by evaluating, planning and implementing effective patient care plans within the guidelines of the standards of practice and operational guidelines.
- Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherpy for patients through multidisciplinary collaboration.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
EDUCATION
- Pharmacy degree (Pharm.D, Bachelor of Pharmacy) from an ACPE accredited school of pharmacy
EXPERIENCE
PHYSICAL REQUIREMENTS
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, kneeling, squatting, twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Rare climbing.
