myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. About this Role
This is a wonderful opportunity for a mission-driven team player to make a real difference in the lives of frail and vulnerable older adults in your community. In this role, you’ll bring warmth, kindness, and compassion to every interaction—whether you’re supporting participants with personal care, accompanying them to appointments, or sharing a smile during a group activity. You’ll work closely with families, caregivers, and our interdisciplinary care team (IDT) to ensure each participant feels safe, seen, and supported, while also staying attentive to any changes in their health or well-being.
